New Service - Your Care Pathway
04th Feburary 2010Age Concern IW is happy to be working on a new pilot for the Isle of Wight Council, entitled Your Care Pathway.
The Pathway will deliver three pilot schemes that will support people to access care services and personal budgets. Age Concern IW is working with the Isle of Wight Advocacy Trust to deliver a pilot that will provide information and advice on services and how care might be funded, as well as helping to prepare support plans and buy services through a personal budget. The other organisations working on the pilot are Citizens' Advice Bureau and Help and Care.
